This is a full-time teleworking position with the expectation to be visiting clients in the lower Prince George's County region in Maryland.

What You Will Do
As a Clinical Care Coordinator, you will provide day-to-day care management and coordination services within the REM Program (Rare and Expensive Case Management). Your clients will be recipients of Medicaid, most of whom are under age 21, and have complex medical needs. Partnering with their family/caregivers, you will work directly with clients on creating an individualized plan of care with goals to help them remain independent in the community setting of their choice. This position is a hybrid of teleworking and client visits in the community setting including client homes, facility settings, school/day programs, and hospitals. This position requires a commitment to and culturally competent approach to Diversity, Equity, Inclusion and Belonging.

The salary for this position is $70,000-$72,000 and comes with all of our full time benefits. This position requires the CCM certification within two years of employment, with a $1500 stipend upon completion.
